SL Coaching Industries Disclosure and Consent 

I, {name-1} (your typed name in the “Name” field above),  acknowledge that SL Coaching Industries will participate in discovering the belief systems that govern my life, and through various Life Consulting methods created by it, help to change those beliefs to ones that I desire to have. Life Consulting is a term SL Coaching Industries defines as: helping guide an individual through the pathways of their heart (thoughts, feelings, and emotions) in order to discover a heightened awareness of self. It is the goal of Life Consulting to establish a personal identity (thoughts, feelings and emotions towards oneself) that is foundationally based in love and acceptance. The goal of Life Consulting is not to fix me but rather to empower me through love and support (believing in me and for me) in order for me, the client, to make a change. Life Consulting, simply said, is coming alongside an individual and walking hand in hand together in the journey of discovering the greatness of oneself. 

SL Coaching Industries believes that each and every human being has been created as a powerful creator, no matter their gender, social, or economic status. SL Coaching Industries believes that every individual’s experience of this world is based out of their own personal perspective developed through years of thought and belief. SL Coaching Industries believes that many of an individual’s negative circumstances result because of personal beliefs, although not in every circumstance. Life circumstances are subject to change as the beliefs within an individual change. Thus, each and every individual has and wields the power to own and take responsibility for their lives. SL Coaching Industries believes that shame, guilt, and condemnation towards self will only yield destructive actions and reactions within life. Love and acceptance of oneself, no matter the individual’s choices that they have made, are the key to a life of emotional health and wealth.

I understand that SL Coaching Industries, its employees, contractors, and agents are not licensed physicians or psychologists or providers of medical or psychological services. The services offered by SL Coaching Industries are educational and informational only and constitute alternative and complementary services to healing arts services licensed by Virginia. The services that SL Coaching Industries offers are not licensed by Virginia. SL Coaching Industries’s education, training, experience, and other qualifications are as follows: ministry school and training; first-hand, practical experience and familiarity receiving emotional health and heart healing coaching with various coaches and licensed counselors; first-hand, practical experience receiving EMDR therapy with licensed practitioners, and other mentorship/coaching from industry leaders.

I am not engaging SL Coaching Industries for any medical or psychological services. I understand that SL Coaching Industries does not diagnose, treat, or claim to cure any medical or psychological condition, and that SL Coaching Industries’s services are not designed to replace conventional treatment methods of medical or psychological conditions. I also understand that SL Coaching Industries does not offer therapy for emotional or mental disorders. I am responsible for my own health care decision-making by obtaining any necessary consultations with appropriately licensed health care professionals such as physicians and psychologists.

I understand that during this Program (as further described herein), I will be confronting my inner feelings and emotions, which could cause emotional pain and distress. I agree to seek medical assistance or psychotherapy or any other appropriate physical or mental diagnosis and treatment from a practitioner duly licensed in the state wherein you reside (such as a licensed medical doctor or licensed psychologist) if I find that these distressing aspects create a danger for myself or for others. I acknowledge that I have not been hospitalized for any psychiatric condition within the last ten (10) years, nor have charges been brought against me based on my behavior.

I understand that SL Coaching Industries is — to the best of its ability and within its permitted scope of services — doing what it can to help me achieve more freedom in my life. I recognize that SL Coaching Industries cannot guarantee results or any specific outcomes from our work together. I am solely responsible for any action taken based on my interpretation of any information presented.

I understand that SL Coaching Industries has the right to refuse to continue delivering services at any time for any reason whatsoever and will refund my payment in full for the portion of unused services.

I understand that if I receive Life Consulting from SL Coaching Industries, it is committed to respect my disclosed information, but not to complete confidentiality. For example, in the event that SL Coaching Industries is led to believe that I may harm myself and/or others, they will take the appropriate steps to deter this from happening, including the disclosure of my personally identifiable information and confidential information to the appropriate authorities.

I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the advice, opinions, or help that I might receive from SL Coaching Industries.

SL Coaching Industries may communicate with me over the phone, email, social media platforms, or over videoconferencing technology. Anticipated benefits include improved access to SL Coaching Industries and allows for SL Coaching Industries and me to communicate while in different physical locations. Potential risks include gaps of failures in communication, complicating SL Coaching Industries’s services, notwithstanding reasonable efforts to ensure the quality and reliability of transmitted information. There may be limitations to image quality or other electronic problems that are beyond the control of SL Coaching Industries. Despite reasonable security measures, online communications can be forwarded, intercepted, or even changed or falsified without my knowledge by third parties.

Our Commitment

Our commitment to you during our Program is to devote ourselves completely to doing all we can to help you discover what it is that you believe about yourself and your life. It is our highest goal to create an environment in which you feel safe, seen, known, heard, and loved, thus feeling comfortable enough to be honest and transparent about all of your thoughts and feelings.

Your Commitment

Expect homework assignments. After most lessons, homework assignments will be given to you. It is imperative that you do all the homework. All of it is purposed to help you in discovering your personal thought life. The greater your awareness of this, the greater chance you have at changing it.

What we expect from you is first and foremost a desire for change. If you don’t want to embrace change, there’s no need to understand what you believe and why you believe it. Knowing these things without desire for change will only create greater problems for you. Without desire for change, there is no journey to go on. Every journey begins and ends. It starts in one place and ends in another. Come Alive Course will demand that you go somewhere. Where do you want to go? In order to go there you must desire change. If desire for change is not the number one desire in your heart at the moment of signing this then our time together will not be well spent. Without change as your greatest desire you will not face the many fears that govern how you think and what you believe.

Commitment to honesty and transparency in regard to your thoughts, feelings, and emotions is a must.

It begins with you being honest and transparent with you. Don’t lie to yourself. As long as you lie to yourself, you’ll be lying to us and thus our time together will not yield progress.

Homework is designed to be a major key to help unlock your beliefs. It’s meant to inspire you and throw you forward. Making homework a priority is making you a priority. Homework will always tell us something about you. What you answer and what you don’t answer reveal something. What is easy to do and what is difficult to do also reveals something. Homework is a stepping-stone for success in this journey.

I understand that this Program contains the Intellectual Property of SL Coaching Industries and its constituents. Accordingly, SL Coaching Industries hereby provides me with a limited license to use the Intellectual Property in furtherance of and during the course of the Program. Such license will terminate upon the completion of the Program. I agree to not use, copy, or disseminate any of this material for any purpose whatsoever at any time without the express written consent of SL Coaching Industries.

I commit to “running to” the SL Coaching Industries team and not from them when I feel stuck.

I commit to making all payments on time and understand that it is my responsibility to honor my commitment regardless of external financial choices/priorities. There are no cancellations or refunds. In the event of your failure to make payments, including as a result of declined payments, you have a 5-day grace period to update your payment information, as necessary, and make payment.

Payment Options:

I understand that my life is my responsibility and nobody can do the work for me or fix me, that I alone can affect change in my life by my choices. 

What you will be Receiving in the Program  (with exception of the discovery call): 

• Online access for 6 months to hours of training that includes step-by-step tools and processes for getting free internally. 
• Access for 6 months to weekly Live Group Coaching via Google Meet for group coaching and accountability 
• Access to community support and encouragement through Google Meet and website. 
• Access for 6 months to scheduled live group coaching video sessions via Google Meet (including pre-recorded, on-demand sessions) to get additional coaching and support as needed. 
• Participation in a 6-topic course to understand you and your pain and help adopt processes to improve these emotionally broken areas.

 I knowingly, voluntarily, and intelligently decide to receive the services described above, and I knowingly, voluntarily, and intelligently assume all risks involved in the same. As a result of my assumption of these risks, I agree to release, hold harmless, indemnify, and defend SL Coaching Industries and its agents from and against any and all claims which I (or my representatives) may have for any loss, damage, or injury arising out of or in connection with use of the services described above, or arising out of or in connection with referral to other practitioners or merchants for delivery of any services. This release includes, without limitation, direct, indirect, incidental, special, consequential, exemplary, incidental, special, or punitive damages, personal injury/wrongful death, lost profits, good will, use data or other intangible losses (whether or not we have been advised of the possibility of such damages)) or damages arising from or relating to the Consent or this Agreement, even if advised of the possibility of such loss or damages, and whether under negligence or some other theory of liability.

Because some states do not allow the exclusion or limitation of liability for consequential or incidental damages, in such states, liability is limited to the fullest extent permitted by law. Notwithstanding the above, SL Coaching Industries’s sole liability for any reason to me, and my sole and exclusive remedy for any cause or claim whatsoever, shall be limited to the amount paid by me for services or other service provided by SL Coaching Industries, and, must be brought within one (1) year since the event giving rise to such action occurred. I understand and agree that my participation with respect to receiving services is predicated upon my waiver of any right to participate in a class action suit for any losses or damages resulting from the same.

Cancellation Policy: Cancellations within the 24-hour window will not be refunded. All other reschedules are case by case basis. Refunds depend on your banking institution’s processing time and therefore, could take between 3 and 5 business days to appear in your account.
